Neck pain is almost universal in modern life — and almost entirely treatable with the right physical therapy approach. Whether it's stiffness on waking, pain radiating into your arm, or tension headaches that track up from your shoulders, I treat these presentations daily across Boca Raton and South Florida.
Why Neck Pain Is So Common in South Florida
The cervical spine is one of the most mobile structures in the body, supporting a 10–12 lb head through an enormous range of daily motion. It's also under constant load from screen time, desk work, driving, and the forward-flexed postures that dominate modern life.
The most common diagnoses I treat:
Cervical muscle strain: Acute or chronic overload of the cervical and upper trapezius musculature. The most common presentation — particularly in desk workers and drivers.
Cervical radiculopathy: A compressed or irritated nerve root producing pain, numbness, or tingling radiating down the arm. Often from disc herniation (C5-6 and C6-7 are most common) or foraminal stenosis.
Cervicogenic headaches: Headaches originating from cervical joint dysfunction — often misdiagnosed as migraines. Distinguished by unilateral pain that begins in the neck and radiates forward.
Cervical facet syndrome: Irritation of the small joints between cervical vertebrae, producing neck pain and stiffness that's worst after sleep or prolonged static positioning.
What Physical Therapy Does for Neck Pain
Physical therapy is first-line treatment for virtually all cervical pain presentations. The approach depends heavily on the diagnosis:
Manual therapy and joint mobilization: High-value for cervical facet syndrome and cervicogenic headaches. Specific grade III–IV mobilization of restricted cervical segments often produces immediate pain relief and range of motion improvements.
Cervical traction: Either manual (hands-on) or mechanical, traction reduces intradiscal pressure and opens the foramina — particularly beneficial for radiculopathy.
Deep neck flexor strengthening: The longus colli and longus capitis — the deep stabilizers of the cervical spine — are almost universally inhibited in chronic neck pain. Reactivating these muscles with chin tuck progressions is foundational.
Scapular stabilization: The cervical spine cannot function independently of the shoulder girdle. Weak lower and middle trapezius muscles create mechanical overload on the cervical region. Shoulder blade stability work is part of every neck program I run.
Postural education: Sustainable changes to workstation setup, phone use, and sleeping position have more long-term impact than any single exercise.
Cervical Radiculopathy: Do You Actually Need Surgery?
The short answer for the vast majority of patients: no. Cervical radiculopathy — even with true nerve compression confirmed on MRI — has a natural history of resolution in most cases, and physical therapy accelerates that resolution significantly.
Research consistently shows that conservative management (PT, manual therapy, activity modification) produces outcomes equivalent to surgery at 1-year follow-up for most cervical disc presentations. Surgery remains appropriate for progressive neurological deficits (worsening weakness), severe myelopathy, or failure after 6+ months of conservative care.

How long does neck PT take?
Most acute neck pain resolves in 4–6 weeks. Chronic presentations or radiculopathy typically require 8–12 weeks of consistent PT.
Is massage enough for neck pain?
Massage provides short-term relief but doesn't address the underlying strength and mobility deficits. PT combines manual work with specific exercises and postural training for lasting results.
Can PT help headaches caused by neck problems?
Yes — cervicogenic headaches respond very well to cervical mobilization and deep neck flexor training. I often see headache frequency cut in half within 4–6 sessions.
