About Dr. Ezra & Empower Fitness
Who is Dr. Ezra Miller?
Dr. Ezra Miller is a Doctor of Physical Therapy (PT, DPT) and the founder of Empower Fitness in Boca Raton, FL. He specializes in concierge in-home physical therapy, return-to-sport rehabilitation, longevity training for active adults, and performance nutrition coaching. He holds a Doctorate in Physical Therapy and brings years of clinical experience across outpatient, sports medicine, and private practice settings.
What makes Empower Fitness different from a regular PT clinic?
Three things: time, attention, and setting. At a traditional clinic, you might see a PT for 20–30 minutes while they're managing 3–4 other patients — often delegating your care to a PT assistant or aide. At Empower Fitness, every session is 60 minutes of 1-on-1 care with Dr. Ezra, delivered in your own home. No waiting rooms, no parking, no shared equipment, and no rushing. Your program is built specifically for you — and Dr. Ezra is personally accountable for your results.
What areas does Empower Fitness serve?
Empower Fitness primarily serves Boca Raton and surrounding South Florida communities including Delray Beach, Boynton Beach, Deerfield Beach, Pompano Beach, and Highland Beach. If you're unsure whether your address is within the service area, call or text for confirmation.
Is Dr. Ezra licensed and insured?
Yes. Dr. Ezra Miller holds an active Florida Physical Therapy license and carries full professional liability insurance. He is a member of the American Physical Therapy Association (APTA).
In-Home Physical Therapy
What is in-home physical therapy?
In-home physical therapy is exactly what it sounds like — a licensed Doctor of Physical Therapy comes to your home and provides treatment in your own space. You receive the same clinical-grade assessment, manual therapy, and therapeutic exercise programming you'd get at a clinic, with the added benefit of being in your own environment (where your recovery needs to function anyway).
What conditions can be treated at home?
Almost any musculoskeletal or orthopedic condition can be treated in-home, including: back pain, neck pain, shoulder injuries (rotator cuff, impingement), knee pain, hip pain, post-surgical rehab (joint replacement, ACL, rotator cuff repair), plantar fasciitis, balance disorders, osteoarthritis, sports injuries, and more. If you're unsure whether your condition is appropriate for home PT, the free consultation call will help clarify.
Do you bring all the equipment you need?
Yes. Dr. Ezra arrives fully equipped — resistance bands, therapy tools, portable equipment, and everything needed for a complete session. In most cases, your body and living room are the most important equipment.
Do I need a referral or prescription to start in-home PT?
No referral required. Florida law allows direct access to physical therapy — you can schedule with Dr. Ezra without first seeing your primary care physician or getting a prescription.
How many sessions will I need?
It depends entirely on your condition, goals, and how your body responds to treatment. Some clients with acute injuries see significant improvement in 4–6 sessions. Others with complex post-surgical rehab or chronic conditions may work with Dr. Ezra for 2–3 months. After your initial evaluation, you'll get a realistic timeline with clear milestones.
What is a typical in-home PT session like?
Sessions run 60 minutes and are structured around your individual program. A typical session includes: a brief check-in and reassessment, hands-on manual therapy (joint mobilization, soft tissue work, stretching), therapeutic exercise progression, functional movement training, and a home program review. Every session builds on the last — there's no repetitive 'one-size-fits-all' routine.
Pricing & Insurance
Do you accept health insurance?
Empower Fitness operates on a private-pay (out-of-pocket) model. This means Dr. Ezra is not bound by insurance-dictated session lengths, visit limits, or billing codes — allowing him to provide true 60-minute sessions focused entirely on your goals rather than documentation requirements.
Can I use my HSA or FSA to pay?
Yes. Physical therapy is a qualified medical expense, so HSA (Health Savings Account) and FSA (Flexible Spending Account) funds can be used for sessions with Dr. Ezra. Many clients find this significantly offsets the cost.
Is private-pay PT more expensive than going through insurance?
Not necessarily, when you compare true costs. A co-pay of $40–$60 per clinic visit, multiplied across 2–3 visits per week for 6–8 weeks, can easily exceed $500–$1,000 — for shorter sessions with less direct DPT time. Empower Fitness sessions are priced transparently, with no surprise bills, no deductibles, and no prior authorization delays. Most clients find the value substantially higher.
Is there a fee for the initial consultation?
The initial 15-minute phone consultation is completely free. This call helps Dr. Ezra understand your situation and confirm that in-home PT is the right approach for you — no commitment required.
Return to Sport
What is return-to-sport physical therapy?
Return-to-sport PT is a specialized rehabilitation pathway designed for athletes (recreational or competitive) recovering from injury, surgery, or performance decline. It goes beyond basic pain relief to focus on sport-specific movement patterns, strength benchmarks, neuromuscular control, and cleared return criteria. Dr. Ezra works with a wide range of athletes including runners, golfers, tennis and pickleball players, swimmers, cyclists, and CrossFit athletes.
How do you know when an athlete is ready to return to their sport?
Dr. Ezra uses validated return-to-sport criteria based on the specific sport and injury — including limb symmetry testing, functional movement screens, strength benchmarks, and sport-specific movement assessments. There's no guesswork. Clearance is based on objective criteria, not just how you 'feel'.
Can you help with golf and tennis injuries specifically?
Yes. Golf and tennis are among the most common sports treated at Empower Fitness. Common golf injuries include low back pain, hip impingement, wrist and elbow injuries, and shoulder pain. Tennis and pickleball injuries frequently involve lateral epicondylitis (tennis elbow), rotator cuff strains, knee stress, and ankle sprains — all treatable with in-home PT.
Longevity Training & Fitness
What is longevity training?
Longevity training is a proactive, science-backed approach to building the physical foundation for a long, active life. Dr. Ezra's longevity programs focus on the four pillars most predictive of healthspan: strength (especially lower body), cardiovascular fitness (VO2 max), balance and stability, and mobility and flexibility. It's not about looking good — it's about being able to move well, live independently, and enjoy activity in your 70s, 80s, and beyond.
Who is longevity training for?
Primarily for adults 45+, though anyone focused on long-term physical performance can benefit. It's especially powerful for: active adults who want to stay competitive in tennis, golf, or pickleball; people approaching retirement who want to maximize their healthspan; individuals post-rehab who want to go beyond 'recovered' to 'thriving'; and high performers who treat their physical health like a strategic investment.
Is longevity training the same as personal training?
No. Longevity training at Empower Fitness is led by a Doctor of Physical Therapy, not a personal trainer. The programming is grounded in clinical movement science, injury prevention, and physiological research — not gym aesthetics. Dr. Ezra also integrates rehabilitation principles, meaning he identifies and addresses movement compensations before they become injuries.
Getting Started
How do I book an appointment?
The first step is a free 15-minute phone consultation. You can book via the contact form at empowerfitnesspt.com/contact or by calling/texting directly. Dr. Ezra will personally reach out within one business day.
How quickly can I be seen?
Most new clients can be scheduled for their first in-home evaluation within 2–4 business days of their consultation call.
What should I have ready for my first visit?
Wear comfortable, moveable clothing. Have any relevant imaging (X-rays, MRI reports) or surgical notes available if applicable — though these are not required. Dr. Ezra will conduct a thorough assessment regardless. Prepare a short history of your symptoms: when they started, what makes them better or worse, and what your goals are.
What if I'm not sure whether PT is the right next step?
That's exactly what the free consultation is for. Tell Dr. Ezra what's going on — he'll give you an honest assessment of whether in-home PT is appropriate, or whether you might need imaging or a different type of provider first. There's zero pressure to book a paid session.
