Common Wrist and Hand Conditions I Treat
Carpal tunnel syndrome (CTS): Compression of the median nerve at the wrist producing numbness, tingling, and weakness in the thumb and first 2–3 fingers. Extremely common in desk workers, cyclists, and padel/pickleball players.
De Quervain's tenosynovitis: Inflammation of the tendons at the thumb-side of the wrist (APL and EPB). Produces pain with thumb movement and a positive Finkelstein's test. Common post-pregnancy and in racket sport athletes.
Wrist sprains: Ligament injuries from falls on outstretched hand — the most common wrist injury mechanism.
Trigger finger: Stenosing tenosynovitis producing catching or locking of a finger during flexion. Often responds to splinting and tendon gliding exercises before progressing to injection or surgery.
Dupuytren's contracture: Progressive fibrotic cord formation in the palmar fascia causing finger contractures. PT can address early-stage cases and post-surgical rehabilitation.
Carpal Tunnel: Non-Surgical Management
Surgery is not the first line of treatment for carpal tunnel syndrome — and for many patients, conservative management produces durable resolution without it. Evidence-based conservative approaches:
Night splinting: Neutral wrist splint during sleep reduces median nerve compression during the hours when most CTS symptoms are worst. Effective for moderate presentations.
Nerve and tendon gliding exercises: Specific mobilization exercises for the median nerve and carpal tunnel contents reduce adhesions and improve nerve mobility. Highly effective when performed consistently.
Activity and ergonomic modification: Identifying and correcting the load factors driving the compression — keyboard position, grip force, vibrating tools, prolonged flexed wrist posture.
Cervical and thoracic mobilization: The "double crush" phenomenon (proximal nerve sensitization amplifying distal symptoms) is underrecognized in CTS. Cervical treatment can dramatically reduce CTS symptoms even without direct wrist treatment.
Post-Surgical Wrist Rehabilitation
After carpal tunnel release, wrist fracture fixation, or tendon repair, rehabilitation is essential for restoring full strength and function. In-home PT after hand surgery is particularly effective because the precise movements required — fine motor function, grip strength progression, scar management — are easily performed in the home setting and benefit from frequent sessions in the early weeks.

Can PT cure carpal tunnel without surgery?
For many patients — particularly mild to moderate CTS without significant thenar muscle wasting — conservative PT produces lasting resolution. Severe cases with persistent weakness may require surgical release for full recovery.
How do I know if my wrist pain is carpal tunnel?
Classic CTS: numbness or tingling in thumb, index, middle, and half of ring finger. Worse at night and with prolonged wrist flexion (driving, phone use). Positive Phalen's and Tinel's signs on exam. A formal assessment provides a definitive picture.
How long does wrist PT take?
Acute wrist sprains: 4–6 weeks. De Quervain's: 6–10 weeks. Carpal tunnel (conservative): 8–12 weeks. Post-surgical: varies by procedure, typically 8–16 weeks.
