Falls are the leading cause of injury-related death among adults over 65 in the United States — and South Florida, with its large senior population, sees this acutely. Why Balance Declines With Age
Balance is maintained by three systems working together:
Vestibular system: The inner ear provides information about head position and movement. Vestibular function declines with age and can be disrupted by conditions like benign paroxysmal positional vertigo (BPPV).
Somatosensory system: Proprioceptive input from joints and muscles — especially the feet, ankles, and hips — tells the brain where the body is in space. Peripheral neuropathy, arthritis, and reduced sensation impair this.
Visual system: Vision provides spatial orientation cues. Reduced visual acuity, cataracts, and night vision changes all impair balance.
When any one of these systems is impaired, the other two compensate. When two are impaired, balance becomes unreliable — especially on uneven surfaces, in low light, or during dual-task situations.

Identify specific fall hazards: loose rugs, poor lighting, bathroom grab bar placement, bed height, furniture layout
Practice the specific transitions that cause falls: getting out of bed at 3am, navigating to the bathroom in low light, stepping over the shower threshold
Build confidence in the movements that have become fear-provoking — which is itself a fall risk factor (fear of falling reduces activity, which reduces the conditioning needed to prevent falls)
What a Fall Prevention Program Looks Like
Evidence-based fall prevention PT combines several elements:
Ankle and hip strengthening: The ankle strategy (small perturbations) and hip strategy (larger corrections) are the two primary balance recovery mechanisms. Strengthening both is foundational.
Progressive balance training: From wide-stance static balance to tandem stance, single-leg balance with eyes closed, and balance on foam/unstable surfaces.
Gait training: Addressing the slow, shuffling gait pattern that increases fall risk. Step length, cadence, and arm swing work.
Dual-task training: Most falls happen when attention is divided. Practicing balance while performing a cognitive task (counting, answering questions) prepares the nervous system for real-world demands.
BPPV treatment: If dizziness is a contributing factor, canalith repositioning maneuvers (Epley) can resolve BPPV in 1–2 sessions with dramatic improvement in balance and vertigo.
Common Questions
How many fall prevention PT sessions are needed?
Significant balance improvement typically requires 8–12 sessions. A maintenance program of 1–2 sessions per month sustains gains long-term.
Does Medicare cover fall prevention PT?
Yes — physical therapy for balance deficits and fall prevention is covered under Medicare Part B when medically necessary. I can advise on documentation requirements.
What causes sudden balance problems in seniors?
Sudden onset should be evaluated medically to rule out stroke, medication side effects, or vestibular pathology. Gradual decline in balance is common with normal aging and highly responsive to PT.
