Understanding Your Hip Replacement Recovery
After THR, you'll have one of two surgical approaches — posterior or anterior — each with different precautions during the healing phase:
Posterior approach: The classic approach, most common. Hip precautions apply for 6–8 weeks: no flexion past 90°, no crossing legs, no internal rotation. These prevent dislocation while the posterior capsule heals.
Anterior approach: Increasingly popular. Generally no hip precautions (or very limited ones) because the posterior capsule is not cut. Faster early return to function.
I coordinate with your surgeon's specific protocol from day one. Knowing your approach and precautions — if any — determines everything about early exercise prescription and activity modification.

Functional Goals by Phase
Week 1–2: Safe ambulation with walker. Bed transfers. Stair negotiation if applicable. Beginning hip and glute activation exercises within precautions.
Week 3–6: Transition to cane. Hip abductor and extensor strengthening. Balance work begins. Walking distance increasing.
Week 6–12: Independent ambulation without assistive device. Return to driving (typically 6–8 weeks). Progressive hip strengthening. Return to low-impact activity.
Month 3–6: Return to recreational activity: golf, swimming, cycling, doubles tennis. Higher-impact activity with surgeon clearance.
Common Questions
When can I walk without a cane after hip replacement?
Most patients transition off a cane by 6–8 weeks if progressing well with PT. Rushing this transition increases fall risk — I'll tell you exactly when you're ready based on your strength and gait pattern.
Can I sleep on my side after THR?
Not initially with posterior approach (dislocation risk). With anterior approach, side-lying with a pillow between knees is typically allowed early. I provide specific sleep positioning guidance based on your surgical approach.
What activities are permanently restricted after hip replacement?
Modern implants are far more durable than previous generations. High-impact activities (running, basketball, soccer) are generally discouraged for implant longevity. Golf, swimming, cycling, doubles tennis, and most daily activities are fully permitted.
