What to Expect After Total Knee Replacement
The typical TKR recovery timeline:
Week 1–2: Hospital discharge (often day 1–2 now with modern protocols). Managing swelling, beginning weight-bearing with walker. Pain peaks around day 3–5 then gradually improves.
Week 3–6: Transitioning from walker to cane. Range of motion progressing toward 90+ degrees. Beginning more active exercise.
Week 6–12: Walking independently without assistive device for most patients. Driving typically resumed by 6 weeks (for right-side surgery). Continuing strength and balance work.
Month 3–6: Return to most recreational activities. Full outcome — complete pain resolution and maximum strength — often takes 12 months.

Range of motion (ROM): Achieving and maintaining 90+ degrees of flexion is the primary early goal. Scar tissue forms rapidly in the first weeks — early, consistent ROM work prevents contractures that are difficult to reverse.
Quad activation: The quadriceps are inhibited by post-surgical swelling and pain. Regaining quad control early prevents the muscle atrophy and knee instability that slow recovery.
Gait training: Progressing from walker to cane to independent walking with normal mechanics — eliminating the antalgic gait pattern that compensatory habits can ingrain.
Swelling management: Elevation, ice protocol, compression, and early pumping exercises to reduce the swelling that limits ROM and function.

When can I drive after knee replacement?
Most patients can resume driving 4–6 weeks after left-side TKR (reaction time returns). Right-side TKR: 6–8 weeks, or per your surgeon's clearance. I can test your emergency braking reaction time before you return to driving.
What exercises should I do at home after TKR?
Ankle pumps, quad sets, heel slides, straight leg raises, and short arc quads are typically the first exercises. I provide a progressive home program that advances week by week based on your specific recovery.
Is it normal to have knee pain 3 months after replacement?
Yes — complete pain resolution takes 6–12 months for most patients. Persistent significant pain at 3 months should be communicated to your surgeon and PT to rule out complications.
